# BENEFITS APPEAL AND MANDATORY RECONSIDERATION LETTER

## 1. Purpose and parties
I am Hannah Lewis, National Insurance number QQ 12 34 56 Q, claimant for Personal Independence Payment, decision reference PIP-44128. The decision dated 2 April 2030 awarded 2 daily-living points and no mobility points. This letter requests a mandatory reconsideration and is not yet the tribunal appeal.
## 2. Facts, scope and terms
The assessor recorded that I could prepare food, wash and dress reliably without help. My multiple sclerosis causes fatigue, tremor and falls: on most days I need supervision to prepare a simple meal, a shower seat and prompting to dress. My partner's statement, GP letter and occupational-therapy report explain the frequency and safety issues.
## 3. Process and responsibilities
For preparing food I rely on the descriptor for needing supervision or assistance; for washing and bathing I need an aid and supervision; for dressing I need prompting and physical assistance. The activities cannot be completed safely, repeatedly, to an acceptable standard or within a reasonable time. I ask the decision-maker to apply the PIP Regulations descriptors to the evidence.
## 4. Evidence, records and safeguards
The assessment report says I denied falls and uses a visit on my best day, but the contemporaneous falls diary and GP record show three falls in March. I enclose the report, diary, medication list, OT assessment and partner statement, and consent to relevant DWP medical-data processing. Please tell me if further evidence is required.
## 5. Review, escalation and outcome
Please revise the award and issue a mandatory reconsideration notice with reasons. If the decision remains wrong, I will appeal to the First-tier Tribunal (Social Entitlement Chamber), normally using SSCS1 within one month of the MR notice. I request that any tribunal appeal is stayed only if the DWP confirms a lawful revised decision.

I will continue reporting changes and attending reasonable assessments. This request does not ask for a favourable decision based only on diagnosis: it explains functional impact and the statutory reliability test. My representative may receive correspondence under the authority enclosed.
