# CARE HOME ADMISSION AGREEMENT

Parties: Willowbank Care Home Ltd, Peter James Evans and Laura Evans
## 1. Parties and purpose
Willowbank Care Home Ltd (Company No. 07651248), CQC registration 1-7654321098, of 10 Church Road, York YO24 1AB, admits Peter James Evans to Room 18 from 15 January 2029. Laura Evans, his daughter and deputy for property and financial affairs, signs the financial provisions; Peter’s wishes and decision-making rights remain central.
## 2. Scope, price and subject
The room is single occupancy with an en-suite, call system and access to the communal lounge and garden. The weekly fee is £1,185, payable every Monday in advance, including accommodation, meals, laundry, ordinary nursing support and activities. Personal toiletries, hairdressing, chiropody and privately requested transport are charged at the published rates.
## 3. Operating duties
The service includes the care-plan support assessed on admission: assistance with washing and dressing, diabetes monitoring, falls prevention and prescribed medication administration. Willowbank must maintain MAR charts, care notes, incident reports and a current risk assessment, and must notify Laura promptly of a serious incident or material change in need.
## 4. Compliance, records and controls
Peter has a right to dignity, privacy, family contact, advocacy, religious observance and participation in decisions. Any restriction must be necessary, proportionate and recorded under the Mental Capacity Act 2005; a deprivation of liberty requires the applicable authorisation. Safeguarding concerns are reported under the Care Act 2014 and CQC requirements.
## 5. Term, ending and remedies
Laura pays the fee from Peter’s nominated account. If the local authority or NHS Continuing Healthcare funds part of the placement, the parties will provide accurate information and pay only the assessed contribution; the home must not misrepresent self-funding as a permanent obligation. A six-week fee review follows changes in care needs, subject to reasonable notice.
## 6. Liability and reservations
Either party may end the placement on 28 days’ notice, or sooner if a lawful hospital discharge, alternative placement or serious risk requires it. Willowbank must consult Peter and Laura, prepare medication and records for transfer, and give a written statement of account. Non-payment triggers a written arrears plan before court action, except in an emergency.
## 7. Governing law and signatures
English law governs and the courts of England and Wales have jurisdiction, without limiting CQC complaints, local-authority safeguarding, NHS processes or Peter’s statutory rights. Willowbank’s manager Helen Brooks, Peter and Laura sign on 15 January 2029 after being offered independent advice and an accessible copy.
