# CARER AUTHORITY LETTER FOR DAY-TO-DAY COMMUNICATIONS

Parties: Margaret Lewis and Hannah Lewis
## 1. Purpose and parties
I, Margaret Lewis of 6 Brookside Crescent, Swansea SA2 9HT, authorise my daughter Hannah Lewis of 31 Kingfisher Way, Swansea SA1 7DL to help with routine communications while I recover from a fractured hip. I confirm that I understand this letter, have mental capacity to give this authority, and sign it freely on 16 November 2032. Hannah is not appointed as my attorney by this letter.
## 2. Facts, scope and terms
Hannah may speak with Bayview GP Practice about appointments, repeat-prescription queries and general care coordination; Swansea Council about council-tax account CT-441908; Severn Water about account SW-772104; and Redcliff Bank about current account ending 4421. Each organisation may verify my identity directly with me at 01792 555210 before disclosing information.
## 3. Process and responsibilities
The authority is limited to obtaining information, arranging appointments, forwarding correspondence, discussing bills and helping me understand options. Hannah may not withdraw money, make transfers, open or close an account, change a payment mandate, borrow, sell property, consent to treatment, sign a contract, or make a decision requiring a registered Lasting Power of Attorney. I retain all decision-making authority.
## 4. Evidence, records and safeguards
For Redcliff Bank, Hannah may discuss balances, statements, direct debits and card-delivery status, but read-only access is requested and no payment instruction is authorised. For the council and utility provider she may report meter readings, query charges and agree a call-back, but may not accept a liability or settlement without my separate written approval. This letter does not override the policies or legal duties of any recipient.
## 5. Review, escalation and outcome
I enclose copies of my driving licence and Hannah's passport, together with the four account references. Recipients should disclose only information relevant to the stated task, keep it securely, and tell me if a separate consent form, identification check or Subject Access Request is required. Hannah must keep anything she receives confidential and return or securely delete copies when I ask.
## 6. Reservations and practical protections
This authority begins on 16 November 2032 and ends on 31 March 2033, unless I revoke it earlier in writing. I may revoke it by letter, email from my verified address or telephone confirmation followed by written confirmation; organisations should record the revocation and stop relying on this authority. If I lose capacity, this letter does not continue as a substitute for a registered LPA or other lawful authority.
## 7. England and Wales law and completion
This completed fictional letter is governed by England and Wales law and is for practical assistance only. Please confirm acceptance by 30 November 2032, identify any limits, and send important decisions to me as well as Hannah. I ask each recipient to keep an audit note of disclosures, and I reserve the right to complain to the organisation, the ICO or the Financial Ombudsman Service where its applicable jurisdiction permits if this authority is misused.
