CHILD TRAVEL CONSENT LETTER
Important jurisdiction and formalities warning
This fictional letter is an illustrative consent record, not legal advice and not a universally accepted travel authorisation. For a child ordinarily resident in England and Wales, the person travelling must check parental responsibility, any child-arrangements or prohibited-steps order, passport requirements, airline rules, destination-country entry rules, visa conditions and safeguarding concerns. A consent letter may need notarisation, a certified copy of a birth certificate, translations or additional evidence. It does not authorise travel contrary to a court order or remove a parent's legal rights.
1. Child and consenting parent
I, Rebecca Louise Morgan, of 17 Celandine Street, Manchester M20 4QF, telephone 07700 912184 and email rebecca.morgan@example.test, am the mother of Evan Thomas Morgan, born 8 August 2015. I have parental responsibility for Evan. I give this consent on 28 November 2026 for Evan to travel with his father, Jonathan Miles Morgan, of 41 Grafton View, Manchester M14 6WL, telephone 07700 912185 and email jonathan.morgan@example.test.
I understand that this is a planned holiday and that Jonathan will supervise Evan throughout. Jonathan has paid £2,460 for flights, accommodation and travel insurance, and I understand that this payment is not a maintenance payment or a condition of consent. This consent is limited to the itinerary and dates described below. It is not a general permission for Jonathan to relocate Evan, change Evan's residence, obtain a new passport, or make decisions beyond those reasonably required for the journey and ordinary care.
2. Journey and itinerary
Evan and Jonathan intend to travel from Manchester Airport, England, to Lisbon Airport, Portugal, on 19 December 2026 on flight Northstar NS218, departing at 10:20 a.m. They intend to return from Lisbon to Manchester on 27 December 2026 on flight Northstar NS219, arriving at 9:05 p.m. The booking reference is NS7Q4L2, and the return ticket is confirmed.
They will stay at Rua das Oliveiras 28, 3rd Floor, Lisbon 1100-412, Portugal, from 19 to 27 December. The accommodation telephone number is +351 21 555 0184. The planned activities are sightseeing, a family meal and age-appropriate museums. Jonathan will not take Evan to another country without Rebecca's written agreement and any consent required by law.
If a flight is cancelled or a short delay makes the stated arrival time inaccurate, Jonathan may arrange reasonable alternative transport and accommodation, notify Rebecca as soon as possible, and keep records. If the itinerary changes materially, he will seek written agreement where practicable and check the requirements of the relevant authorities.
3. Care, documents and medical authority
Jonathan will carry Evan's passport, a copy of Evan's full birth certificate, this signed letter, evidence of the parents' relationship, travel insurance and the itinerary. Rebecca will provide a copy of any court or child-arrangements document relevant to travel, and both parents will check that Evan's passport remains valid for Portugal. The adults understand that officials may request original or certified documents and that this letter cannot guarantee admission.
Evan has a mild peanut allergy and carries an adrenaline auto-injector in the labelled blue travel pouch. Jonathan will keep it accessible, follow the written medical plan, and carry the emergency contact card. Evan's GP is Dr Amina Shah, Oakwell Medical Centre, 9 Wilmslow Road, Manchester M20 3BW, telephone 0161 555 2640. Jonathan may consent to routine urgent medical assessment and treatment during the trip where delay would risk Evan's health, and he will inform Rebecca promptly. This paragraph does not replace the consent required by a clinician or local law.
4. Communication and emergency arrangements
Jonathan will message Rebecca after departure, on arrival, every evening at approximately 7:30 p.m. Lisbon time, and before the return flight. Evan may speak with Rebecca by video call when practical, with flexibility for meals, activities and sleep. Jonathan will tell Rebecca promptly about illness, accident, loss of documents, contact from authorities or any other significant event.
If Jonathan cannot care for Evan, he will contact Rebecca first and then Clara Naomi Morgan, 62 Fairfield Crescent, Manchester M21 8TA, telephone 07700 912186. Clara is an emergency contact only and is not appointed as a guardian by this letter. Jonathan will contact emergency services in an immediate emergency and cooperate with the relevant embassy, airline, insurer or child-protection authority.
5. Confirmation and signatures
I confirm that I have read the itinerary, understand the proposed supervision and give the limited consent stated above voluntarily. I have not been pressured, and I understand that this letter may need to be replaced if dates, destination, accompanying adult or legal circumstances change.
