# SAMPLE COUNCIL COMPLAINT LETTER

Parties: Leah Morgan and Northshire Council

## 1. Status of this document

Leah Morgan of 8 Birch Street, York YO1 6AA complains to Northshire Council about three missed assisted waste collections on 17 April, 1 May and 8 May 2029. Her household has an agreed assisted-collection marker.

## 2. Parties and subject

The bins were presented by 6.30 am at the agreed collection point, but no collection or explanation was recorded. Leah has attached photographs, dates and the reference NC-8841 from her telephone reports.

## 3. Facts and request

She asks the service to collect the accumulated waste within 48 hours, check the marker and explain why the missed collections were not notified. She also requests a reasonable adjustment: email and plain-language correspondence.

## 4. Checks and evidence

Leah asks for a stage-one response within ten working days, the officer responsible and the service standard relied on. She does not ask the council to disclose another resident's personal information.

## 5. Procedure and deadline

The council should acknowledge the complaint, investigate route records and contractor data, and give a reasoned response with corrective action. Any urgent health risk should be referred to the appropriate team.

## 6. Rights and remedies

If the response is inadequate, Leah will use the council's stage-two process and any applicable Local Government and Social Care Ombudsman route. She reserves statutory environmental and equality rights.

## 7. Records and signature
