# DRIVING LICENCE APPEAL

I enclose that report and the GP notes, subject to appropriate handling of health data. The evidence does not establish that I presently meet every licensing standard, but it supports a fresh, careful assessment rather than an assumption.
## 4. Evidence, records and safeguards
Please disclose the medical questionnaire, clinician advice, decision record, relevant factual material and the exact medical standard applied. Please also confirm whether the decision is a revocation or refusal following a licence application, its effective date, the review process and the deadline for an appeal. I understand DVLA may seek further reports and that a successful challenge is not automatic.
## 5. Review, escalation and outcome
Pending review I accept that I must not drive, and I will notify my insurer and employer as appropriate. I ask DVLA to arrange an independent medical review or reconsideration after receiving the cardiology evidence. If a restricted or conditional return is legally available, I will comply with any required monitoring and reporting; I do not request an exception outside the licensing rules.
