# EMERGENCY MEDICAL CONSENT AND INFORMATION AUTHORITY
## Purpose and limits
This fictional document is signed at Oakbridge Primary School, 12 Carlton Rise, Harrogate HG2 8LT, on 2 September 2027 under school reference OBS-MED-ELL-27. It gives the school a clear record of parental responsibility, relevant health information and a limited authority for urgent medical arrangements for one child during the stated period. It is not a lasting power of attorney, court order, blanket consent to elective treatment, consent to a clinical procedure that a clinician considers inappropriate, or authority to override the child's own legal rights and capacity. Clinicians must assess the child's best interests, consent requirements, safeguarding information and current medical facts. Emergency services may act under their legal powers when delay would risk serious harm.
## Child and parents
The child is Elliot James Mercer, born 21 February 2016, illustrative NHS number 999 999 999 9. This repeated number is deliberately fictional for this worked example and is checksum-valid under the NHS number check-digit method; it must not be used as a real identifier. Before any clinical record is opened, shared or relied upon, the school and clinician must verify Elliot's actual NHS number and identity directly with Oakbridge Medical Practice, 4 Station Road, Harrogate HG1 1AA, telephone 01423 555 166, or Harrogate District Hospital, Lancaster Park Road, Harrogate HG2 7SX. Elliot lives at 51 Willowbank Avenue, Harrogate HG2 7QJ. His school admission number is OB-2416 and his class teacher is Ms Helen Brook.

We are not asking the school to decide a disagreement between people with parental responsibility. If the school is told of a later court order or safeguarding direction, it must follow that lawful information and seek advice.
## Health information for urgent care
Elliot has a severe allergy to amoxicillin and other penicillin antibiotics. His reaction on 11 May 2024 involved hives, facial swelling and breathing difficulty and was treated at Harrogate District Hospital. The school must record the allergy prominently, tell emergency clinicians and never give a penicillin antibiotic unless a qualified clinician with the relevant information makes and documents that decision. Elliot has no known food allergy. He has mild exercise-induced asthma and uses a blue salbutamol inhaler, two puffs through a spacer when prescribed and needed. The inhaler is kept in the labelled medical cabinet and a spare is in his school bag. The school should follow the current signed asthma plan dated 28 August 2027.
Elliot's current weight recorded by Oakbridge Medical Practice on 28 August 2027 is 31.4 kilograms. That figure is for identification and clinical context, not a dosing instruction. The school must not calculate or administer a medicine by using this document alone. His GP is Dr Aisha Rahman. The parents will provide an updated medication form if a prescription, dose or plan changes. Elliot takes no daily medicine at school and has no known seizure disorder, diabetes or heart condition as at the signing date.
If Elliot suffers breathing difficulty, facial swelling, collapse, a severe injury, suspected poisoning, an altered level of consciousness or another immediately serious event, the school must call 999, follow the emergency operator's instructions and tell the emergency clinician about the penicillin allergy and asthma. The school may administer a prescribed reliever or emergency medicine only in accordance with the current written plan, training and lawful authority. It must not delay an emergency call while trying to reach either parent.
## Authority given to the school
From 2 September 2027 until 31 August 2028, Oakbridge Primary School may obtain first aid, call NHS 111, arrange transport to an emergency department and provide relevant medical information to a registered clinician when Elliot needs urgent assessment or treatment during school hours, a school trip, an authorised sports activity or an after-school activity supervised by the school. The headteacher Marion Ellis, deputy head Peter Osei, school nurse Clara Denton and any trained staff member supervising Elliot may communicate this information and accompany him until a parent or lawful carer arrives.
The parents consent, to the extent the law permits, to urgent examination, diagnostic assessment, first aid and treatment recommended by the responsible clinician where waiting for a parent would create a material risk of harm or unnecessary suffering. This authority covers treatment at an NHS hospital, ambulance service, NHS urgent-treatment centre or registered GP service. This form gives no advance parental consent to a blood transfusion. That limitation does not prevent clinicians or emergency services from providing treatment that is legally authorised or required, including treatment under emergency powers or a court order. The responsible clinician retains clinical judgment and safeguarding duties. This form does not give advance consent to elective surgery, dental treatment, vaccination, a clinical trial, psychiatric admission, or a course of treatment that can safely wait for parental discussion. For non-urgent care, the school must contact Rachel or Jonathan and the clinician must obtain the consent required by law.
The school must make reasonable efforts to call Rachel first on 01423 555 701 and Jonathan second on 01423 555 702. If neither answers, it must call Rachel's sister Priya Mercer, 17 Birkdale Walk, Knaresborough HG5 0NF, telephone 01423 555 703, then Jonathan's brother Lewis Mercer, 6 Daleside Close, Ripon HG4 1TW, telephone 01765 555 704. Priya and Lewis are emergency contacts only and have no parental responsibility or independent authority to consent to non-urgent treatment. The school must leave a voicemail where possible, record the time and continue the clinically appropriate emergency process.
Parents may be reached by email at the addresses above, but an email is not a safe substitute for a 999 call and email alone does not authorise treatment. An email notice about Elliot or a proposed change to this authority is effective only when the intended recipient acknowledges receipt in a reply or the school independently verifies receipt by telephone using an existing contact number. A delivery confirmation or the absence of a delivery-failure message is not acknowledgment. If an important non-urgent email is not acknowledged within one school day, the school must telephone the parent and send a letter by first-class or recorded post to the relevant address above. For an urgent event, staff must telephone Rachel or Jonathan, follow clinical advice and call 999 immediately where there is an immediate risk to life or serious injury; staff must not wait for email. The school must confirm identity by its existing contact records before relying on an instruction that changes this document.
## Practical records and aftercare
For every incident the school will record the date, time, place, observed symptoms, first-aid steps, medicine and dose if any, staff present, calls made, advice received, hospital destination, parent arrival and handover. It will provide that record to Rachel and Jonathan securely within two school days, subject to safeguarding and data-protection obligations. The school will notify the parents as soon as reasonably practicable after emergency services are called and will not publish Elliot's medical information.
The parents will provide the school with the inhaler, signed asthma plan, allergy alert, verification of Elliot's actual NHS number and any updated hospital or GP letter. Rachel will check the expiry date of the inhaler on 2 September 2027 and again on 2 January 2028. The school will tell the parents if a medicine is missing, expired, damaged or used. A parent must collect Elliot or nominate a responsible adult after treatment unless the clinician or safeguarding authority directs otherwise.
This authority expires automatically at 23.59 on 31 August 2028, when Elliot leaves Oakbridge Primary School, when a parent gives written replacement instructions accepted by the headteacher, or when a court or safeguarding authority makes a conflicting order, whichever occurs first. It does not continue merely because an old copy remains in a school file. The parents will review it at the start of each term and immediately after any allergy, diagnosis, medication, address or contact-number change.
## Execution
Rachel and Jonathan confirm that the information was accurate to the best of their knowledge on 2 September 2027 and that they have explained to Elliot, in age-appropriate language, how adults will help him in an emergency. They understand that this document cannot require a clinician to provide treatment that is unsafe or outside the clinician's professional judgment.
