# INFORMAL CARE AGREEMENT
Date: 1 June 2029
Parties: Grace Evans and Leo Evans
## 1. Parties and purpose
Grace Evans of 14 Cedar Lane, Exeter EX2 4HT is Leo Evans's daughter and informal carer. Leo, aged 78, lives at the same address and has capacity to agree this practical arrangement; it is not a contract of employment or a regulated care-provider contract.
## 2. Money and rights
From 10 June 2029 Grace will visit each morning and evening, help Leo prepare meals and prescriptions, accompany him to appointments, and coordinate paid home-care visits. Leo retains control over his home, finances and personal decisions.
## 3. Duties and operation
Grace must respect Leo's dignity, privacy and choices, record medication prompts accurately, and raise concerns about falls, neglect, abuse or changing needs with Leo, the GP or adult social-care team. She must not restrain Leo or handle his bank card without specific authority.
## 4. Consent and management
Leo will reimburse agreed travel and grocery costs against receipts, up to £85 per month. No wage, fee or gift is promised; any benefits, Carer's Allowance or means-tested support must be considered separately and truthfully.
## 5. Ending and remedies
Grace may access only the information needed for care, keep keys secure and not share medical details except with Leo's consent or where safeguarding or an emergency makes disclosure necessary. Leo may appoint an attorney under a lasting power of attorney separately.
## 6. Legal reservations
Either person may pause or end this arrangement by written or clearly communicated notice, particularly if it becomes unsafe or burdensome. They will seek an assessment from the local authority if needs increase, and will review the plan on 1 September 2029.
## 7. Signatures
This agreement records intentions and does not replace a care plan, capacity assessment, safeguarding duty or professional advice. It is governed by England and Wales law; both parties sign freely and keep a copy.
