# MEDICAL RESEARCH CONSENT FORM（MEDICAL RESEARCH同意書）

## 1. 当事者と目的
7 Beech Avenue, Nottingham NG5 3RPのOliver James Reedは、North Trent NHS Foundation TrustのDr Leah Morganがleadするfictional RESTORE-LUNG observational studyに招待されます。studyはasthma admission後のrecovery dataを調べるもので、treatment、diagnosis、personal medical benefitをpromiseせず、Oliverは理由なくdeclineできます。
## 2. 対象・対価・範囲
説明と質問の時間の後、Oliverはbaseline interview 1回、relevant NHS recordの5年間review、18か月間のfollow-up questionnaire 2回にconsentします。optional biomarker sub-studyのblood sampleはmain studyに不要で、Oliverはそのsampleについて別にconsentをtickしましたが、断ってもcore studyを失いません。
## 3. 実施上の義務
予想されるburdenはvisit約45分、optional blood drawのminor bruising、illnessについて話す際のdiscomfortです。experimental medicineはadministerしません。continued participationに関わるnew informationはresearch teamが説明し、urgent clinical helpはstudy teamを待たずordinary NHS routeで求めます。
## 4. 法令・記録・管理
TrustはUK GDPRとData Protection Act 2018に基づき、public interestのtaskとscientific research safeguardのためidentifiable informationをprocessします。analysisではstudy codeを使い、keyはauthorised staffに限定します。resultはOliverをidentifyせずaggregateでpublishできます。safeguarding、serious risk、law、regulatorが求める場合はconfidentialityにlimitがあります。
## 5. 期間・終了・救済
participationはvoluntaryで、Oliverはresearch officeへ連絡していつでもwithdrawできます。NHS care、benefit、employment、Trustとのrelationshipに影響しません。withdrawal後はnew data collectionとfuture contactを止めますが、既に使用した、またはresearch integrityに必要なcoded data・sampleはretainできます。law上deleteできる範囲はTrustが説明します。
## 6. 責任と留保
paymentはありませんが、receiptを伴うcompleted in-person visitごとに35ポンドまでreasonable travel expenseをreimburseします。Trustはnegligent harmのindemnity arrangementを維持し、NHS complaintとresearch-ethics routeを説明します。ordinary study inconvenienceのcompensationはguaranteeされず、本consentはstatutory rightをwaiveせずclinical contractを作りません。
## 7. 準拠法と署名
Oliverはparticipant sheetに署名してcopyを受け、Dr Morganはresearch teamを代表して2029年5月20日に署名します。studyはHealth Research Authority approval、favourable Research Ethics Committee opinion、clinical governance、UK GDPR、safeguarding requirementに従います。England and Wales lawを適用し、本formはmedical decision-making authorityをtransferせずconsentを記録します。
