# PET ADOPTION AGREEMENT（PET ADOPTION契約）

## 1. 当事者、目的およびlegal route
Moorland Animal Rescue CIO（Charity No. 1198402、3 Station Yard, York YO24 1AB）は、4歳の避妊済み雌lurcherであるMabelを11 Holly Close, York YO10 4NPのHannah Lewisへplaceします。Rescueはknown history、vaccination record、behaviour assessmentをdiscloseしました。
## 2. 事実、scopeおよびsupporting evidence
Hannahはsigning時に£180のcontributionを払い、collection時にownershipとday-to-day careを受けます。ただし以下のwelfare・return obligationがあり、breedingのsaleでも、discloseしたbehaviourが全て消えるguaranteeでもありません。
## 3. request、義務およびsafeguard
Hannahはsuitable food、exercise、secure accommodation、microchip registration、annual vaccination、prompt veterinary treatmentを提供します。Animal Welfare Act 2006を守り、publicでMabelをcontrolし、dog walkerやboarding serviceには事前permissionを取ります。
## 4. record、reviewおよびcommunication
Mabelをbreed、dog fightingに使い、abandon、sell、give awayしてはいけません。lost animal、serious bite incident、material welfare concernは48時間以内にRescueへnotifyします。move、landlord restriction、financial difficultyはsafe solutionのため早期に相談します。
## 5. time limit、escalationおよびoutcome
Rescueは最初の3か月にagreementによるreasonable welfare visitを一度行い、veterinary confirmationを求められます。Hannahはcontact detailを正確に提供し、UK GDPRに基づきwelfare follow-upに必要なpersonal dataのholdingへconsentできます。consentなしのpublic marketing imageは使いません。
## 6. 留保および実務上のprotection
安全にkeepできなくなった場合、HannahはまずRescueまたはapproved rehoming partnerにreturnをofferし、private transferをしません。cruelty、neglect、unlawful abandonmentならRescueはurgent protective actionを求められますが、通常のdisagreementだけでrepossession powerは生じません。
## 7. England and Walesのlawと署名
England and Wales lawを適用し、そのcourtsにjurisdictionがあります。noticeはhand delivery、post、welfare@moorlandrescue.exampleへのemailをalternative methodとして送れます。Hannahとauthorised trusteeがcare pack受領後に署名し、statutory dutyによるanimal welfareを最優先とします。
