# ペットケア・犬の散歩契約書（記入例）

当事者： Amelia RossおよびGreen Paws Care Ltd
## 1. 当事者と目的
Green Paws Care Ltd（Company No. 13690127）は、5 Willow Close, York YO24 1GAでAmelia Rossのdog Brambleとcat Juniperをcareします。regular serviceはAmeliaの旅行中、2029年6月10日から6月30日までです。
## 2. 対象・対価・範囲
Green Pawsは平日に45分のsolo dog walk 1回、毎晩のcat visit、feeding、fresh water、指示どおりのmedication、写真付きdaily messageを提供します。BrambleはAmeliaが認めたenclosed area以外ではleadにつなぎ、Juniperは屋内に置きます。
## 3. 実施上の義務
Ameliaはfood、harness、medication、veterinary detail、emergency contact、鍵2本を提供します。vaccinationとbehaviour informationが正確で、bite risk、allergy、escape historyをdiscloseすると確認します。Green Pawsは鍵をsecureに保管しcopyせず、service後2日以内に返します。
## 4. 法令・記録・管理
animalがinjury、illness、missing、immediate dangerなら、Green Pawsはemergency contactに連絡しYork Vets Ltdへ連れて行けます。welfareが危ういdelayを避けるため500ポンドまでのnecessary first aid・veterinary treatmentをauthoriseし、それを超えるnon-urgent treatmentは可能ならAmeliaの同意を得ます。veterinary costはAmeliaが負担します。
## 5. 期間・終了・救済
feeはdog walk 1回22ポンド、cat visit 1回18ポンドで、weekly invoiceをBACSで7日以内に支払います。Ameliaは24時間前noticeでcancelでき、late cancellationは50%ですが、illness・emergencyで不合理なら除きます。Green Pawsは通知後trained carerをsubstituteできます。
## 6. 責任と留保
各当事者はreasonable careを尽くします。Green Pawsは200万ポンドpublic-liability insuranceを維持し、Animal Welfare Act 2006とlocal requirementを守ります。pre-existing conditionや正確なinstructionに従った結果には責任を負いませんが、negligence・deliberate defaultには責任を負います。
## 7. 準拠法と署名
双方は7日前written noticeで終了でき、serious welfareまたはsafety concernならimmediate terminationできます。イングランド法を適用し、courts of England and Walesがjurisdictionを持ちます。Amelia、Green Pawsのdirector Nisha Patel、指定keyholderが2029年6月6日に署名します。
