## 1. 目的と当事者
incidentは2030年10月28日09:40、Bristol office loading areaで発生しました。warehouse assistantのNoah Reedがsealed equipment boxを運ぶ際loose pallet strapにつまずき、left kneeを打ちました。first aiderはbruiseを記録し、urgent-care後、翌日はdesk dutyが可能でした。
## 2. 事実、範囲および条件
witnessはPriya ShahとcontractorのLewis Grantです。CCTVをpreserveしました。palletは09:20にsupplierがdeliveryし、strapがmarked pedestrian routeを横切り、delivery checklistは即時removalを求めていませんでした。equipment defectやdeliberate actは確認されていません。
## 3. 手続と責任
areaをcordon offし、strapをremoveし、floorをcheckし、supplierへloading bay使用を指示しました。Noahにはmedical support、appointmentのpaid time、temporary seated taskをofferしました。本factual reportはblameをassignせず、comment希望ならaccountをaddします。
## 4. 証拠、記録および保護措置
initial root-causeはinadequate delivery segregationとchecklist gapで、temporary loading teamへのbriefing failureもcontributeしました。corrective actionはrevised induction、barrier、supervisor sign-off、weekly route inspectionで、ownerとdue dateをsafety registerに置きます。
## 5. review、escalationおよびoutcome
Health and Safety ManagerはRIDDOR 2013をassessしました。現在のfactsではbruiseとone-day adjustmentはspecified injuryではなく、accidentはpresently reportableではありません。fracture diagnosis、7 consecutive days超のincapacity、別のreportable circumstanceなら再assessします。
## 6. 留保と実務上の保護
report、photo、CCTV reference、first-aid entry、witness account、risk assessmentをsecureにretainします。personal dataはsafety、insurance、employment、lawful investigationの担当者だけにshareします。2030年11月12日にactionをreviewしNoahとconsultします。
## 7. England and Walesの法と完了
England and Wales lawを適用します。本reportはliabilityのadmissionではなく、新factがあればHSE notificationを妨げません。Health and Safety ManagerのOliver Grantが2030年10月29日にsignします。
