# MEDICAL NEGLIGENCE LETTER OF CLAIM

Date: 4 July 2029

Parties: Hannah Price and Southmoor NHS Trust

## 1. Status of this document

I write under the Pre-Action Protocol for the Resolution of Clinical Disputes about treatment at Southmoor General Hospital on 8 February 2028. I was admitted with abdominal pain and underwent surgery. This open Letter of Claim is sent to Southmoor NHS Trust and its claims handler.

## 2. Parties and subject

The preliminary allegation is that the surgical team failed to recognise and respond to a documented fall in blood pressure and failed to investigate worsening pain and fever promptly. A CT scan was obtained approximately 19 hours later, showing a leak that required a second operation.

## 3. Facts and request

I suffered sepsis, a 12-day critical-care admission, further surgery and a scar. I remain fatigued, cannot work my former nursery role full-time and require follow-up. Causation and prognosis are matters for independent experts; this letter is not a medical conclusion.

## 4. Evidence and checks

I will obtain reports from a colorectal surgeon, microbiologist, pain specialist and care or occupational expert as appropriate. Preliminary special damages are £18,460 for lost earnings, travel, care and treatment, subject to a schedule; general damages and future loss are reserved pending evidence.

## 5. Procedure and deadline

Please preserve records, audit trails, observations, drug charts, imaging, consent material, incident reports, staff rotas and policies. Please confirm the Trust's legal identity, indemnity arrangements and whether the records can be supplied in an accessible electronic format.

## 6. Rights and remedies

Please acknowledge within 14 days and provide a Letter of Response within the Protocol timetable, identifying admissions, denials and reasons. Limitation is not agreed or waived; I understand court proceedings, expert evidence and NHS Resolution processes have specific requirements.

## 7. Records and signature

This letter is sent to promote a fair investigation and settlement, not as a Part 7 claim or statement of final valuation. I reserve interest, costs, contributory-negligence issues and all remedies. I sign on 4 July 2029 after preserving consent and confidentiality.
