To the Data Protection Officer, North Bristol NHS Trust, Information Governance Team, Southmead Road, Bristol BS10 5NB: I, Aisha Rahman of 33 Elm Street, Bristol BS3 4LT, make this subject access request under Article 15 UK GDPR. My date of birth is 14 May 1986 and NHS number is 487 221 9031.
## 2. Facts, scope and supporting evidence
Please provide copies of my Trust records from 1 January 2023 to the date of your response, including clinical notes, referral and discharge letters, test results, imaging reports, medication records, appointment notes and correspondence that identifies me. This request concerns my cardiology admission from 3 to 8 August 2029 and related GP referrals.
## 3. Requests, duties and safeguards
Please search the electronic patient record, scanned correspondence and relevant archived record. I prefer secure electronic PDF delivery, with a separate explanation if any record is withheld or redacted. This request is for access, not a demand that the clinical record be altered; I will make a rectification request separately if necessary.
## 4. Records, review and communication
I enclose a certified copy of my passport and council-tax letter showing my address. Please tell me promptly if further reasonable identity or authority evidence is needed. The Trust should process the request lawfully, protect third-party confidentiality and explain any exemption relied upon.
## 5. Time limits, escalation and outcome
Please confirm receipt and respond without undue delay and in any event within one month of receiving the necessary information, subject to any lawful extension for a complex request. If the records are held by my GP rather than the Trust, please identify the controller and forward or clarify the correct route.

If I disagree with a withholding or delay, please provide the internal review and complaints route and tell me how to complain to the Information Commissioner's Office. Nothing in this request waives confidentiality, clinical safety or any separate access, rectification or litigation right.
