# NAME CHANGE NOTIFICATION LETTER

## 1. Status of this document
Please update my customer records from Leah Morgan to Leah Morgan-Price. I am writing to Westborough Bank plc, account ending 7804, from 11 Willow Parade, Cardiff CF10 2AB. My customer number is WB-184902.
## 2. Parties and subject
I executed a deed poll on 3 July 2029 and attach a certified copy together with proof of address. The change is a personal name change; it does not change my identity, beneficial ownership, account mandate or tax status.
## 3. Facts and request
Please update statements, online banking display, correspondence and the name used for debit-card replacement. Do not close or re-open the account. If the bank requires its own form, witnessing, identity check or original document, please tell me exactly what is needed and why.
## 4. Evidence and checks
Please retain the old name only where required for audit, fraud prevention, legal record or account history, and restrict access under your privacy notice. I do not consent to using this request as a marketing update or sharing the deed poll unnecessarily.
## 5. Procedure and deadline
I understand that a deed poll does not itself amend a passport, driving licence, Companies House entry or another organisation's record. I will make separate applications where required and this letter is not a direction to alter any statutory register outside the bank's control.

Please confirm within 15 working days whether the update is complete, which systems were changed and whether any payment or standing order could be affected. If you refuse, provide written reasons and your complaints route; I reserve any rights under the account terms and applicable law.
