## 1. Purpose and parties

I, Amelia Rose Carter of 14 Willowbank Road, Leeds LS6 2QJ, write to Northbridge Analytics Limited, my employer, to notify you that I changed my legal name to Amelia Rose Morgan by an unenrolled deed poll dated 1 September 2032. I remain the same employee, born 6 February 1991, with payroll reference NA-1842, and this request does not change my employment contract, salary or start date of 3 May 2021.

## 2. Facts, scope and terms

Please update my employment, payroll, pension and benefits records from Carter to Morgan after verifying the enclosed certified copy of the deed poll. My current address remains 14 Willowbank Road and my bank account is unchanged. I ask Payroll to use Amelia Rose Morgan on the payroll run closing on 23 September 2032, subject to its ordinary verification process, rather than treating this letter as authority to alter any bank mandate.

## 3. Process and responsibilities

I enclose the certified deed-poll copy, a copy of my current UK passport showing Amelia Rose Carter, and a utility bill dated 28 August 2032. My passport has not yet been replaced, so the difference between the passport and the deed poll is expected. I can provide the original for inspection by an agreed appointment, but ask Northbridge not to retain it and to return any original by recorded delivery.

## 4. Evidence, records and safeguards

Please amend my payslips and internal directory, issue any corrected P60 required for the relevant tax year, notify the pension administrator and benefits providers where necessary, and tell me which records require a separate external application. My National Insurance number is not repeated in this general letter; I will provide it through Northbridge's secure HR portal if required. Please restrict access to the documents to staff with a legitimate HR or payroll need.

## 5. Review, escalation and outcome

Please confirm in writing by 26 September 2032 that the change has been recorded, or identify any specific missing evidence and the team responsible for resolving it. I will check the September payslip and, when issued, the relevant P60, and will report an error promptly. No request is made to backdate a tax filing or alter historical records that must lawfully retain my former name.

## 6. Reservations and practical protections

This notification is made under the ordinary evidence requirements of each organisation and is not an assertion that every public record changes automatically. I understand that an unenrolled deed poll is not an application for enrolment in the public record, and I have not requested enrolment. Northbridge should use my former name only where necessary to link historic records, with the new name used for future correspondence.
