## 1. Status of this document

Priya Nair of 6 Orchard Yard, York YO10 4LP complains to Northshire NHS Foundation Trust about the cancellation and delayed rebooking of her dermatology appointment. Her NHS number and date of birth are supplied only through the Trust's secure channel.

## 2. Parties and subject

The appointment on 9 April 2029 was cancelled by text on the morning of attendance. Priya was told a new appointment would follow within ten days, but by 27 May she had received no date and symptoms had worsened.
