# EXAMPLE: NOTICE OF SUSPENSION
Date: 12 February 2028
From: Fenland Logistics Limited
To: Daniel Price
## 1. Facts
From Fenland Logistics Limited, Unit 4 Fen Road, Peterborough PE1 4QH, to Daniel Price, Warehouse Supervisor, dated 12 February 2028. This letter confirms the decision made by Priya Nair, Operations Director.
## 2. Decision
You are suspended on full basic pay from 13 February while we investigate an allegation that you authorised an unrecorded forklift maintenance override on 8 February and asked Aisha Khan not to log it.
## 3. Reasons
Suspension is a neutral measure, not a disciplinary finding. It is necessary because you supervise the relevant records and witnesses; alternative supervision was considered and was not workable for the short investigation.
## 4. Process and rights
You must remain available between 9.00 am and 5.00 pm, not attend Fenland premises, contact the named witnesses about the allegation or alter records. You may contact your union representative and receive normal contractual benefits.
## 5. Records and continuing matters
The investigator, Marcus Bell, will invite you to a fact-finding meeting within five working days, provide the allegation and relevant evidence, and allow you to respond. The company will review suspension every 14 days and lift it when no longer necessary.
## 6. Closing
You may request a reasonable adjustment, raise a welfare concern with People Team and be accompanied at any formal disciplinary hearing. If a hearing is warranted, a separate outcome letter will explain findings and appeal rights under the ACAS Code.
## 7. Acknowledgement
Please acknowledge receipt to People Team by 15 February. Contact Harriet Cole if you believe a condition is unclear. Yours sincerely, Priya Nair, Operations Director; signed: ____________________.
