# ORGAN DONATION ADVANCE DECISION LETTER

## 1. Purpose and parties
I, Elin Roberts of 3 Pen-y-Bryn, Cardiff CF5 2LA, born 22 August 1987, NHS number 943 817 6201, record these voluntary wishes on 14 January 2032. I have registered on the NHS Organ Donor Register as consenting to donation of organs and tissue for transplantation, and my registration confirmation dated 8 January 2032 is kept with this letter. This record supplements, rather than replaces, the official register.
## 2. Facts, scope and terms
I consent to donation of my heart, lungs, liver, kidneys, pancreas, small bowel, corneas, skin, bone, heart valves and tendons where clinicians consider them medically suitable. I consent to donation for transplantation and, if a transplant is not possible, separately consent to the use of donated tissue for approved medical research or education where the applicable consent process permits it. I do not impose a religious or geographic condition.
## 3. Process and responsibilities
My wishes are subject to clinical assessment, lawful authority and the practical circumstances at death; they are not a direction to clinicians to continue treatment that is clinically futile. I understand that organ donation may occur only after death is established under applicable clinical rules and that some organs or tissue may not be usable. I have not made an advance decision to refuse life-sustaining treatment in this document, and this letter must not be read as one.
## 4. Evidence, records and safeguards
I have discussed the decision with my spouse, Rhys Roberts, and my sister Carys Morgan. I ask the medical team to tell Rhys first, at 07700 900812, and Carys second, at 07700 900813, if contact is practicable. They are asked to support communication of my registered decision, not to replace it or impose a veto; clinicians should explain any legal or clinical limitation sensitively.
## 5. Review, escalation and outcome
I authorise the NHS Organ Donation service and relevant clinicians to check my identity and register status and to share only necessary information with my family and transplant teams. My GP is Dr Helen Price at Riverside Medical Practice, Cardiff. A copy is held in my home document folder marked Donation Wishes, a second copy is with Rhys, and I have asked my GP to note that the register, not this informal copy, is the primary record.
## 6. Reservations and practical protections
I may change or withdraw these wishes by updating the NHS Organ Donor Register and telling my family and GP; the most recent valid record should be checked. I will review this letter on 14 January 2034 or sooner after a major health, family or religious change. The current choice is a voluntary opt-in, and I understand that the applicable deemed-consent frameworks in England and Wales, and any excluded-category rules, do not remove the importance of my clear registered decision.

This completed fictional personal record is governed by England and Wales law and dated 14 January 2032. I sign it freely in the presence of Rhys Roberts and Carys Morgan, who sign only to confirm that I appeared to understand it, not as medical decision-makers.
