# PET ADOPTION AGREEMENT

Parties: Moorland Animal Rescue CIO and Hannah Lewis

## 1. People, purpose and legal route

Moorland Animal Rescue CIO (Charity No. 1198402), 3 Station Yard, York YO24 1AB, places Mabel, a four-year-old neutered female lurcher, with Hannah Lewis of 11 Holly Close, York YO10 4NP. The Rescue has disclosed Mabel's known history, vaccination record and behaviour assessment.

## 2. Facts, scope and supporting evidence

Hannah adopts Mabel for a £180 contribution paid on signing. Ownership and day-to-day care pass on collection, subject to the welfare and return obligations below; this is not a sale for breeding or a guarantee that every disclosed behaviour will disappear.

## 3. Requests, duties and safeguards

Hannah must provide suitable food, exercise, secure accommodation, identification microchip registration, annual vaccinations and prompt veterinary treatment. She must comply with the Animal Welfare Act 2006, keep Mabel under control in public and obtain permission before using a dog walker or boarding service.

## 4. Records, review and communication

Mabel must not be bred, used for fighting, abandoned, sold or given away. Hannah must notify the Rescue within 48 hours of a lost animal, serious bite incident or material welfare concern. A move, landlord restriction or financial difficulty should be discussed early so a safe solution can be found.

## 5. Time limits, escalation and outcome

The Rescue may make one reasonable welfare visit by agreement during the first three months and may request veterinary confirmation. Hannah will provide accurate contact details and may consent to the Rescue holding necessary personal data under UK GDPR for welfare follow-up; no public marketing image is used without consent.

## 6. Reservations and practical protections

If Hannah cannot safely keep Mabel, she must first offer her back to the Rescue or an approved rehoming partner and must not transfer her privately. The Rescue may seek urgent protective action where there is cruelty, neglect or unlawful abandonment; ordinary disagreements do not create a repossession power.

## 7. England and Wales law and signature

England and Wales law governs and courts of England and Wales have jurisdiction. Notices may be hand-delivered, posted or emailed to welfare@moorlandrescue.example as alternative methods. Hannah and the Rescue's authorised trustee sign after receiving the care pack; the statutory duty to protect animal welfare remains paramount.
