# PHASED RETURN TO WORK PLAN

Date: 1 December 2029

Parties: Westbrook Architects LLP and Nadia Lewis

## 1. Parties and purpose

Westbrook Architects LLP of 17 Queen Square, Bristol BS1 4NH agrees this plan with Nadia Lewis, Senior Project Architect, after Nadia's 12-week absence following surgery. It records occupational-health advice and does not require Nadia to disclose a diagnosis beyond what is needed for work.

## 2. Money and rights

Nadia will return on 4 December 2029 for a six-week phased programme: weeks 1–2, three days and 18 hours; weeks 3–4, four days and 27 hours; weeks 5–6, five days and 37.5 hours, subject to review and medical advice.

## 3. Duties and operation

For the first four weeks Nadia will undertake design reviews, client calls and desk-based drawing work but no site visits, lone working or extended deadlines. Westbrook will provide an ergonomic chair, a quiet workstation, flexible start between 09:00 and 10:00 and home working on Wednesdays.

## 4. Consent and management

Nadia will receive full contractual pay for the first four weeks under Westbrook's absence policy, then pay pro-rated to hours worked unless a different entitlement applies. Statutory Sick Pay and holiday calculations will be handled lawfully by payroll.

## 5. Ending and remedies

The line manager will meet Nadia at the end of weeks 2, 4 and 6, with HR invited at Nadia's choice. Either may propose an extension, change or pause; Nadia will not be penalised for following medical advice or requesting a reasonable adjustment under the Equality Act 2010.

## 6. Legal reservations

Nadia will keep in touch by email during any further absence, with fortnightly welfare contact at times she chooses. Westbrook will protect her health information, share it only on a need-to-know basis and make no capability decision without a fair process.

## 7. Signatures

The plan is not a guarantee of medical recovery, a variation of every employment term or a substitute for a fit note. It will be reviewed on 15 January 2030; England and Wales employment law applies and both parties sign after discussion.
