# SOCIAL SERVICES COMPLAINT LETTER
Date: 5 March 2034
Parties: Maya Patel and Northbridge County Council Adult Social Care
## 1. Purpose and parties
Maya Patel submits this Stage 1 complaint on 5 March 2034 to Northbridge County Council Adult Social Care about her father Arun Patel, aged 79, who lives at 18 Cedar Close, Northbridge NB3 7LA. Maya is his daughter and has his consent to communicate; she asks the council to treat her as his representative unless Arun withdraws that consent.
## 2. Facts, scope and terms
Arun was discharged from Northbridge General Hospital on 12 February after a fall. Maya requested a Care Act needs assessment on 13 February and sent the discharge plan, medication list and occupational therapist's concerns on 15 February. No assessment date has been offered by 5 March, although Arun now needs help with washing, stairs and preparing meals.
## 3. Process and responsibilities
On 27 February Arun fell again while trying to reach the bathroom; no fracture was found, but he remained on the floor for approximately 40 minutes before a neighbour arrived. Maya works four days each week and cannot safely provide all care. She asks the council to record the incident as a safeguarding concern and to consider interim support without waiting for this complaint to finish.
## 4. Evidence, records and safeguards
Maya complains that the council has not acknowledged the assessment request, explained any delay, identified a named worker or given a written decision about urgent interim services. She is not asking the council to predetermine eligibility; she asks for a lawful assessment, communication in accessible language and a clear record of the information considered.
## 5. Review, escalation and outcome
She requests an urgent home visit within five working days, a falls and equipment review, a financial-assessment explanation, and a written care-and-support plan if eligible. If the council considers another body responsible, it should identify that body and make a safe referral rather than simply close the contact. Maya can be reached at 01632 555 024.
## 6. Reservations and practical protections
Attached are the hospital discharge plan, medication list, fall chronology, neighbour's statement and Maya's authority to act. Please preserve relevant call notes and allocate a complaints officer independent from the original decision-maker. Maya will provide further information, but neither she nor Arun should be required to repeat distressing facts unnecessarily.
## 7. England and Wales law and completion
This completed fictional complaint is dated 5 March 2034 and asks for a Stage 1 response within the council's published timescale, with reasons and information about Stage 2 escalation. It does not waive Arun's Care Act 2014 rights, the Human Rights Act 1998, safeguarding duties or any urgent route to the NHS or emergency services.
