# TEMPORARY CARE AUTHORITY LETTER
Date: 4 October 2032
Parties: Daniel and Priya Shah; child Noah Shah; temporary carer Leila Shah
## 1. Purpose and parties
We, Daniel Shah and Priya Shah of 22 Fernleigh Avenue, Cardiff CF11 8RA, are Noah Shah's parents and holders of parental responsibility. Noah was born on 18 July 2018 and lives with us. By this letter dated 4 October 2032, we ask Daniel's sister Leila Shah of 9 Orchard Street, Newport NP20 2LS to care for Noah while we are abroad in Canada from 10 October to 24 October 2032.
## 2. Facts, scope and terms
During that period Leila may provide Noah's ordinary accommodation, meals, supervision, clothing and transport within the United Kingdom. She may communicate with Ysgol Glanmor School, attend meetings, sign routine school permission forms and collect Noah from school. This is a practical written authorisation for care and communication, not a transfer of parental responsibility or a court appointment as a guardian.
## 3. Process and responsibilities
Leila may consent to routine GP, dental, optical and pharmacy treatment and may arrange an appointment for an existing condition. If urgent treatment is needed and neither parent can reasonably be reached, she may consent to emergency assessment and treatment to the extent permitted by the clinician. She must give the clinician this letter and our medical information sheet, and must inform us as soon as safely practicable.
## 4. Evidence, records and safeguards
Leila is not authorised to consent to elective major surgery, change Noah's school, alter his residence, make financial or legal decisions, apply for a passport, or take him outside the United Kingdom. She may take Noah on ordinary domestic journeys with our prior notice. ## 5. Review, escalation and outcome
Noah has asthma and carries a blue salbutamol inhaler, two puffs when required in accordance with his action plan; the spare inhaler is in his school bag. His GP is Dr Helen Morris at Riverside Medical Centre, Cardiff, telephone 029 2045 1188. Leila must follow medical and school instructions, keep medicines secure, and call 999 for a life-threatening emergency before trying to reach us.
## 6. Reservations and practical protections
We enclose copies of our passports, Noah's birth certificate, his NHS number in a sealed sheet, the school contact details, the asthma action plan and Leila's identification. Leila must keep the documents confidential, share them only with professionals who need them, and return them to us on 25 October 2032. We will notify the school and GP in advance and will confirm any extension in a fresh signed letter.
## 7. England and Wales law and completion
This completed fictional authority is governed by England and Wales law and expires at 23:59 on 24 October 2032 unless revoked earlier in writing by either parent. It does not override a court order, safeguarding duty or professional clinical judgment. We sign voluntarily on 4 October 2032, ask recipients to verify us by telephone, and authorise Leila to act only within the limits stated above.
