# WORKPLACE ACCIDENT REPORT

## 1. Purpose and parties
Incident: 28 October 2030 at 09:40 in the Bristol office loading area. Warehouse assistant Noah Reed tripped over a loose pallet strap while carrying a sealed equipment box, falling onto his left knee. First aider recorded a bruise; Noah attended an urgent-care centre and was fit for desk duties the next day.
## 2. Facts, scope and terms
Witnesses were Priya Shah and contractor Lewis Grant. CCTV was preserved. The pallet had been delivered by a supplier at 09:20; the strap crossed the marked pedestrian route and the delivery checklist did not require immediate removal. No equipment defect or deliberate act is presently identified.
## 3. Process and responsibilities
The area was cordoned off, the strap removed, the floor checked and the supplier instructed to use the loading bay. Noah was offered medical support, paid time for appointments and a temporary seated task. No blame is assigned in this factual report, and Noah's account will be added if he wishes to comment.
## 4. Evidence, records and safeguards
The initial root-cause analysis is inadequate delivery segregation and a checklist gap, with a contributing failure to brief the temporary loading team. Corrective actions are a revised induction, a barrier, supervisor sign-off and a weekly route inspection, each with an owner and due date in the safety register.
## 5. Review, escalation and outcome
The Health and Safety Manager assessed RIDDOR 2013. On the facts currently known, the bruise and one-day adjustment are not a specified injury and the accident is not presently reportable; the assessment must be revisited if Noah is diagnosed with a fracture, is incapacitated for more than seven consecutive days, or another reportable circumstance emerges.
## 6. Reservations and practical protections
The report, photographs, CCTV reference, first-aid entry, witness accounts and risk assessment are retained securely. Personal data is shared only with those managing safety, insurance, employment or a lawful investigation. The team will review actions on 12 November 2030 and consult Noah.
