# WORKPLACE INJURY REPORT

Parties: Northmere Foods Ltd, Priya Desai and the incident witnesses
## 1. Purpose and parties
Northmere Foods Ltd records that on 14 September 2031 at 09:20, employee Priya Desai, a packing operative, slipped beside Bay 3 in Warehouse 2 at 41 Orchard Road, Peterborough PE1 5QJ. A pallet of chilled sauce had been moved across a leaking refrigeration line; clear liquid remained on the concrete and no warning sign was in place when Priya stepped backwards while carrying a 6 kg carton.
## 2. Facts, scope and terms
Priya fell onto her right hand and reported wrist pain, swelling and a superficial abrasion. First aider Martin Cole applied a cold pack, immobilised the wrist with a support bandage and recorded treatment at 09:31. Priya attended Peterborough City Hospital at 11:05; an X-ray found no fracture. A GP Fit Note dated 15 September 2031 certified 8 days away from work, from 15 to 22 September inclusive, with a review on 23 September.
## 3. Process and responsibilities
Witnesses Aisha Rahman and Ben Wright state that the liquid had been visible for approximately 20 minutes and that the nearest warning sign was stored 18 metres away. Aisha saw the refrigeration contractor's hose being disconnected at 08:55. The shift supervisor, Tom Ellison, was told of a damp patch at 09:02 but did not isolate Bay 3 or place a sign before the incident.
## 4. Evidence, records and safeguards
The immediate cause was an uncontained leak and an unmarked wet surface. The underlying causes were a missed weekly refrigeration inspection on 7 September 2031, unclear responsibility for moving warning signs with pallets, and inadequate supervisor escalation. The investigation file contains the signed statements, the CCTV extract from 08:45 to 09:35, photographs 001–006, the first-aid log, hospital note, Fit Note and risk assessment RA-W2-19.
## 5. Review, escalation and outcome
Because Priya was unable to carry out her normal work for 8 consecutive days, Northmere submitted an online RIDDOR report on 29 September 2031, within the required period for over-7-day incapacitation, under reference RIDDOR-NF-2031-0914-774. The report is not an admission of civil liability. Priya's personal medical information is restricted to HR, the safety manager and professionals who need it for welfare or legal compliance.
## 6. Reservations and practical protections
Maintenance repaired the refrigeration line and tested it on 14 September; Warehouse Manager Elin Jones installed four signs at each warehouse bay by 15 September, added a signed weekly inspection to the maintenance log by 20 September, and briefed all 38 warehouse staff on 21 September. The safety manager updated RA-W2-19 on 24 September. Effectiveness will be checked by an unannounced inspection on 14 October 2031 and reviewed by the Health and Safety Committee on 21 October.

Priya was offered a return discussion, reimbursement of reasonable travel to treatment and access to the employee assistance service; she resumed suitable packing duties on 23 September 2031 subject to medical advice.
