# Universal Credit Sanction — Mandatory Reconsideration

Date: 12 October 2026

Parties and reference: Claimant: Daniel Moore, 7 Hazel Terrace, Nottingham NG7 3PX. To: DWP Universal Credit decision maker through the secure journal. Decision EXAMPLE-SAN-1005.

## 1. Decision and reconsideration request

Dear Decision Maker, I request mandatory reconsideration of the sanction decision dated 5 October 2026 for failing to attend the appointment at 10:00 on 22 September. I received the decision on 7 October and make this request on 12 October, within one month of the decision date. Please confirm the sanction category, start date, duration and calculation, and record that I dispute both the finding of failure without good reason and the resulting reduction.

## 2. Good reason and notification

At 07:40 on 22 September I was taken to hospital with an acute illness and remained under emergency assessment until 16:30. I could not safely attend or arrange a call during that period. My discharge note confirms the attendance and times. I entered a journal message at 18:10 that day, explained the emergency and asked to rebook; I attended the replacement appointment on 29 September. These facts should be considered as my reason for the missed appointment, not merely as a request for sympathy.

## 3. Evidence and requested revision

I enclose the appointment notification, hospital note, same-day journal message and replacement-attendance confirmation. Please review the actual evidence and any work-coach record, revise the decision if good reason is established, remove the sanction and restore any award wrongly reduced. If further information is necessary, identify it specifically and allow me a reasonable opportunity to provide it. I am not asserting that a medical note automatically determines every sanction case.

## 4. Hardship and continuing requirements

The reduced payment leaves me unable to cover essential food and heating. Separately, please explain how to apply for any recoverable hardship payment and the applicable eligibility and repayment conditions. I understand a reconsideration request does not itself suspend the sanction or automatically award hardship support. I will continue meeting my current claimant commitments, or promptly seek an appropriate change if illness prevents compliance, while this dispute is examined.

## 5. Notice, appeal information and contact

Please send a mandatory reconsideration notice with full reasons and the evidence relied on. If you maintain the sanction, explain the appeal route, deadline and how to obtain the relevant papers. Contact me through the secure journal rather than requesting sensitive medical information by ordinary email. Please confirm receipt of each attachment. Yours faithfully, Daniel Moore, signed 12 October 2026.
