# Carer's Assessment Request
Date: 12 October 2026
Parties and reference: Carer: Julia Reed, 8 Fern Avenue, Bristol BS7 8PX. Adult cared for: David Reed, 11 Fern Avenue, Bristol BS7 8PX. To: Bristol City Council Adult Social Care.
## 1. Request for assessment
Dear Adult Social Care Team, I request a carer's assessment under section 10 of the Care Act 2014. I provide unpaid care to my father David, aged 76, who lives in Bristol. It appears that I have needs for support now and in the future. Please assess those needs separately from any assessment of his care needs. I understand the duty to assess does not depend on your initial view of my financial resources or the level of my needs.
## 2. Care provided and sustainability
I provide about 35 hours of care each week: meal preparation, shopping, medication reminders, transport and help with household tasks. I also work 20 paid hours per week. My sleep and attendance at work are affected, and I cannot continue the present arrangement without reliable breaks and an emergency plan. Please assess what I am able and willing to continue doing rather than assuming I will supply unlimited care.
## 3. Outcomes and support to consider
My priorities are to maintain employment, attend my own medical appointments, have one regular evening free and arrange cover if I am ill. Please consider information and advice, suitable breaks, practical training, local carers' services and whether eligible needs require a support plan. Any charging or direct-payment discussion should follow the appropriate assessment; I do not assume that all support is free or that this assessment automatically awards Carer's Allowance.
## 4. Participation, consent and separate adult assessment
Please arrange an initial telephone appointment on a Tuesday afternoon and offer a private part of the assessment so that I can speak freely about my own wellbeing. David agrees to your contacting him to discuss a separate needs assessment, but that is not general authority for me to make decisions for him. Please obtain any necessary consent directly and explain how each person's information will be handled. If the assessment suggests an immediate gap in safe care, please discuss interim arrangements promptly.
## 5. Evidence and written outcome
I enclose a two-week care diary and my work schedule. Please acknowledge the request, give an assessment timetable and identify my contact worker. After assessment, provide the written assessment, eligibility decision with reasons, any agreed support plan and information about review or complaint routes. Please tell me what further evidence is genuinely needed. Yours faithfully, Julia Reed, signed 12 October 2026.
