# Council Housing Transfer Request

Date: 12 October 2026

Parties and reference: Tenant: Jordan Mills, Flat 7, 24 Meadow Road, Bristol BS5 9PX. To: Bristol City Council Housing Allocations Team. Tenancy EXAMPLE-HSG-2407.

## 1. Household and current home

Dear Housing Allocations Team, please register or update my transfer application and assess my housing needs under the applicable allocation scheme. I am a council tenant and live with my eight-year-old daughter in a one-bedroom flat on the second floor, reached by 34 stairs with no lift. My daughter uses the bedroom and I sleep in the living room. I provide these facts for assessment, not as a conclusion that statutory overcrowding has already been established.

## 2. Medical and welfare evidence

A long-term knee condition substantially limits my stair use. My occupational therapist's 28 September report recommends step-free access, and my GP's 30 September letter explains the effect on daily activities. I now need help to carry groceries and sometimes miss appointments because I cannot safely manage the stairs. Please refer the evidence to your medical or welfare assessor and consider how the present accommodation affects both members of the household.

## 3. Requested accommodation and priority assessment

I request a two-bedroom home with step-free access or a reliable lift, within reasonable travelling distance of my daughter's school and my treatment in Bristol. Please assess any reasonable-preference category for medical or welfare needs under Part VI of the Housing Act 1996 where applicable, together with the published scheme's eligibility, qualification and bedroom rules. I understand that evidence does not guarantee a particular band, immediate offer or preferred neighbourhood.

## 4. Attachments, contact and interim options

I enclose the tenancy statement, household details, occupational therapy report and GP letter. Please use the secure housing account for any further medical documents and limit access to staff who need them. I can attend a telephone assessment. Please explain whether a separate transfer route, adaptations assessment or mutual exchange is appropriate while the application is considered; I am not asking to surrender my existing tenancy by making this request.

## 5. Written decision and review rights

Please acknowledge receipt, confirm my application reference and explain the assessment timetable. Send a written decision stating the band or priority, reasons, evidence considered, how to bid where applicable and any review route and deadline. If the application belongs with another team, please identify the correct process rather than treating it as withdrawn. Yours faithfully, Jordan Mills, signed 12 October 2026.
