# Sports Injury Claim — Initial Notification
Date: 19 October 2026
Legal context: England and Wales - fictional worked example; overseas acceptance must be checked where relevant
Parties and reference: Claimant: adult Alice Morgan, 12 Example Grove, Sheffield. To Example Sports Centre Ltd, 8 Example Pitch Lane, Sheffield, organiser and occupier. Incident EXAMPLE-SPORT-1003, adult football coaching session, 3 October 2026 at 10:30.
## 1. Accident and alleged unsafe condition
During an instructed running drill on pitch 2, my foot entered an unmarked depression near the east touchline and I fell onto my left wrist. I was not injured by a tackle or ordinary contact with another player. I allege the organiser failed reasonably to inspect, mark or isolate a foreseeable surface hazard before directing the drill there. I enclose photographs with a scale and witness Maya Cole's details with her permission. Liability is not presumed merely because an injury occurred during sport; duty, breach, causation and any contributory negligence require evidence.
## 2. Injury and present medical information
The emergency department diagnosed a fractured left wrist on 3 October and applied a cast. The treating team's note supports that diagnosis, not an independent opinion on legal causation, prognosis or permanent disability. I continue follow-up and will obtain suitable expert evidence before valuing pain, suffering, loss of amenity or future treatment. Please provide your liability insurer's contact details and consider any reasonable rehabilitation needs without requiring an admission or final settlement before urgent support is discussed.
## 3. Provisional financial losses
The present documented losses are ten unpaid working days from 5 to 16 October at net £120 per day, total £1,200, appointment travel £84 and physiotherapy already paid £160: total £1,444. The employer's letter confirms no sick pay or other wage replacement for those ten days; any later payment will be credited. Receipts and earnings evidence are retained. This is not the total value of the injury claim: general damages and any justified future loss remain unquantified, and no commercial late-payment interest is claimed.
## 4. Evidence preservation and correct procedure
Please preserve pitch inspections, maintenance and earlier defect reports, session plans, incident records and relevant CCTV for the incident period, while taking immediate steps to remove any continuing hazard. This is an initial notification, not a Claim Notification Form or a representation that a pre-action protocol has been completed. My adviser must determine whether the low-value employers' liability/public liability protocol and portal or another procedure applies. Under the general Personal Injury Protocol where applicable, acknowledgment is normally within 21 days and investigation within three months of acknowledgment, not three months automatically from this initial letter.
## 5. Reply, limitation and reservation
Please acknowledge this notification and identify the insurer by 2 November 2026 as a requested administrative date, explaining any missing information. I am willing to consider proportionate ADR once the evidence is sufficient, without accepting a premature full-and-final release. For an adult injury claim the limitation period is ordinarily three years from injury or a relevant later date of knowledge, subject to statutory rules and exceptions; correspondence and negotiation do not stop it. I will obtain timely advice rather than rely on an insurer's timetable. Signed Alice Morgan, 19 October 2026; alice.morgan@example.com.
